ADHD Questionnaires

Tools for the assessment and follow-up

NOTE : All the presented documents are graciously offered by their authors. They are free to use. Please note that these questionnaires and other tools are specially built to be used and analysed by competent professionals.

WARNING! No questionnaire alone makes possible to come up with a diagnosis. These tools facilitate the information gathering process and then guide the procedure to target the problematics and the possible interventions. It is essential for the evaluation process to be done by a competent professional who knows ADHD well because many problematics can mimic its symptoms or be associated to it. In Quebec, you can refer yourself to law 21 about the types of health care professionals that are authorized for the assessment and intervention processes.
In some cases, your health practitioner could ask for a psychological, neuropsychological, speech therapy or occupational therapy evaluation. This is particularly important when a learning disorder, a psychological problematic, a speech disorder or dyspraxia is suspected.

CADDRA ADHD Practice Guidelines offers a full ADHD Toolkit for the assessment and follow-up: CADDRA ADHD Assessment Toolkit (CAAT)
